Fort Wayne native, Dr. Hoffman became a member of Lutheran Health Physicians in August 2010 when her former group, Brooklyn Medical Associates, joined LHP. Dr. Hoffman, a primary care physician, joined BMA in 1995. Dr. Hoffman earned a bachelor's degree from Valparaiso University in 1988 and a medical degree from the Chicago College of Osteopathic Medicine in 1992. She completed an internship at Rush-Presbyterian St. Luke Medical Center in Chicago in 1993 and a residency with Fort Wayne Medical Education Program in 1995. Dr. Hoffman is board certified by the American Board of Family Medicine. She is a member of the American Academy of Family Physicians, the Wilderness Medical Society, the Indiana State Medical Association and the Fort Wayne Medical Society.
