John Jacob, M.D., is a board-certified family medicine specialist who is committed to helping children and adults develop healthy lifestyles and achieve their health goals.

Dr. Jacob provides care for every member of the family, from adolescents to adults. His personalized care includes disease prevention, as well as management for a broad range of chronic diseases, such as high cholesterol and high blood pressure.

Dr. Jacob graduated from medical school at the University of Silesia (Katowice, Poland). He then moved to Sylvania, Ohio, to complete a family medicine residency at ProMedica Flower Hospital.
